Facial Contour and Sagging: Understanding and Treating a Changing Jawline

The facial contour is one of the most visible markers of youth. A well-defined jawline, clean mandibular edges and a clear distinction between the face and neck are instinctively associated with a youthful, dynamic appearance. Over time, this contour gradually blurs. The jawline softens, jowls develop and the boundary between face and neck fades. Understanding the mechanisms behind this process is the first step towards choosing the right treatment. Why the Facial Contour Changes with Age

Loss of facial definition results from three phenomena acting simultaneously. The first is laxity of the SMAS (Superficial Musculo-Aponeurotic System), the deep muscular layer that forms the structural framework of the face. Over the years, the SMAS loses its tone and yields to gravity, dragging down the skin and tissues it supports.

The second phenomenon is volume loss. The fatty compartments of the face atrophy unevenly: the cheekbones hollow, the temples become gaunt, while fat migrates downwards and accumulates around the jowls and beneath the chin. This redistribution of volume accentuates the sagging appearance.

Finally, skin ageing itself plays a role: the skin loses its elasticity (diminished collagen and elastin), thins and can no longer contain the underlying tissues. Resorption of the facial skeleton further compounds the problem by reducing the "frame" on which the soft tissues rest.

Stages of Sagging: From Early Signs to Advanced Laxity

Facial sagging is a gradual process that progresses through stages. Recognising which stage you are at helps determine the most appropriate treatment.

Early Stage (40-45): First Signs

The initial signs are subtle: a slight blurring of the jawline, a hint of nasolabial fold, some loss of cheekbone volume. At this stage, aesthetic medicine delivers excellent results. Hyaluronic acid injections at the cheekbones and along the jaw can redefine the contour. Botox addresses emerging expression lines across the upper third. A chemical peel improves overall skin quality.

Moderate Stage (50-55): Visible Sagging

Sagging becomes clearly apparent: jowls take shape, nasolabial folds deepen noticeably and the neck begins to loosen. Aesthetic medicine alone is no longer sufficient. This is the stage at which a mini-facelift or a MACS lift can deliver significant rejuvenation through a less extensive procedure than a full facelift. These techniques are suited to patients whose sagging is real but still moderate, particularly in the middle third of the face.

Advanced Stage (60+): Generalised Laxity

Sagging affects the entire face and neck. The jawline is markedly blurred, jowls are pronounced, and the neck shows excess skin along with visible platysmal bands. The cervico-facial facelift is the gold standard at this stage. It allows simultaneous treatment of the jawline, jowls, neck and deep folds by repositioning the SMAS at depth. Combined with a platysmaplasty for the neck and fat grafting to restore volume, it represents the most complete approach to facial ageing.

Complete Facial Rejuvenation: A Coherent Approach

Dr Weiss treats the facial contour as part of what he calls "Complete Facial Rejuvenation". This philosophy rests on a simple observation: ageing never affects just one area in isolation. When the jawline sags, the eyes are often showing signs of fatigue as well. Treating the contour without rejuvenating the eyes — or vice versa — creates a visible, unnatural mismatch.

This is why a cervico-facial facelift may be combined with a blepharoplasty for the eyelids, a temporal lift for the brows, or fat grafting for hollow areas. The objective is to treat the face as a whole for a harmonious, natural result. As Dr Weiss puts it: "Less is always better than too much." Every gesture is measured; every correction is proportionate.
