MACS Lift: The Modern Short-Scar Facelift

The MACS Lift, or Minimal Access Cranial Suspension Lift, represents a major evolution in facelift techniques. The fundamental principle of this technique is cranial suspension: loop sutures, anchored to the strong structures of the temporal fascia, lift the sagging facial tissues vertically. This vertical re-suspension is particularly effective because it works directly against the direction of gravity, the very force responsible for facial descent.

The Advantages of a Short Scar

One of the key benefits of the MACS Lift lies in the discretion of its scars. The incision, shorter than that of a traditional cervico-facial facelift, is located exclusively in front of the ear and within the natural crease of the temple. There is no incision behind the ear, which offers several advantages:

  • Less extensive scars that are easier to conceal
  • Preservation of the hairline at the nape of the neck
  • Reduced risk of sensory changes behind the ear
  • Shorter operating time, with the procedure typically lasting 2 to 3 hours

Who Is a Good Candidate for a MACS Lift?

The MACS Lift is primarily suited to patients who present moderate sagging of the mid and lower face. The ideal candidate typically displays:

  • Early loss of jawline definition with developing jowls
  • Sagging cheeks with loss of cheekbone definition
  • Visible nasolabial folds
  • Moderate skin laxity, without significant excess skin on the neck

However, when sagging is more pronounced, particularly in the neck with significant excess skin or visible platysmal bands, the cervico-facial facelift remains the reference technique. MACS Lift and Volume Restoration

One of the great advantages of the MACS Lift is its ability to restore cheekbone volume. The vertical suspension technique lifts sagging fat pads back to their original position at the level of the zygomatic arch. The face regains better-defined cheekbones without necessarily requiring additional volume supplementation.

Nevertheless, in certain cases, the MACS Lift can be combined with fat transfer (lipofilling) to optimise volume restoration, particularly in the temples or hollow under-eye area. This combination delivers an even more harmonious rejuvenation by simultaneously addressing sagging and volume loss.

How Does It Differ from a Traditional Cervico-Facial Facelift?

It is important to understand that the MACS Lift is not a "budget facelift": it is a different technique, designed for different indications. Here are the main distinctions:

The cervico-facial facelift treats the entire face and neck comprehensively. It is indicated for advanced sagging, allows complete neck correction and delivers the most thorough result possible. Its scars are more extensive but remain very discreet.

The MACS Lift favours a more targeted approach to the mid and lower face. Its scars are shorter, recovery is faster (10 to 15 days versus 2 to 3 weeks), and the procedure takes less time. However, its ability to correct the neck is more limited.

Recovery After a MACS Lift

One of the appreciated benefits of the MACS Lift is its faster recovery. Bruising and swelling, which are inevitable after any facial surgery, typically resolve within 10 to 12 days. Most patients can resume social activities after two weeks, using light make-up in the initial period.

The result improves progressively over the weeks following the procedure. The tissues stabilise gradually, and the final result can be appreciated after 2 to 3 months. As with any facelift, the outcome is natural and long-lasting, with benefits that endure for many years.
