Facelift: Which Technique Should You Choose?

With the variety of facelift techniques available today, it is perfectly understandable to feel overwhelmed. Cervico-facial facelift, MACS Lift, mini-facelift, temporal lift... Each technique addresses specific indications and delivers different results. This article will help you understand the characteristics of each, while keeping in mind that only a personalised consultation with your surgeon can determine the technique best suited to your face.

Understanding How the Face Ages

Before comparing techniques, it is essential to understand how the face ages. Facial ageing is not a uniform process: it affects different areas at different rates, and each face ages uniquely depending on its bone structure, skin quality and genetic factors.

Three main mechanisms are at work: volume loss (fat depletion in the temples, cheekbones and cheeks), deep structural sagging (the SMAS descending under the effect of gravity) and skin deterioration (loss of elasticity). Depending on which mechanism predominates, the most suitable facelift technique will differ.

The Cervico-Facial Facelift: The Comprehensive Gold Standard

The cervico-facial facelift is the most complete and most widely performed facelift technique in the world. It simultaneously addresses the jawline, jowls, nasolabial folds, marionette lines and neck laxity.

  • Indication: moderate to advanced sagging of the face and neck
  • Typical age: 50 to 70
  • Duration: 3 to 4 hours, general anaesthesia
  • Recovery: 2 to 3 weeks
  • Result: rejuvenation of 10 to 15 years, lasting 10 years or more

This is the technique of choice when the neck also requires significant correction (excess skin, platysmal bands). Working on the SMAS provides a deep and long-lasting repositioning of the tissues.

The MACS Lift: The Modern Short-Scar Approach

The MACS Lift (Minimal Access Cranial Suspension) delivers significant rejuvenation of the mid and lower face with incisions limited to the pre-auricular area.

  • Indication: moderate facial sagging, without significant neck excess
  • Typical age: 45 to 60
  • Duration: 2 to 3 hours
  • Recovery: 10 to 15 days
  • Result: restored jawline and cheekbone definition, a rested appearance

The MACS Lift is particularly effective at restoring cheekbone volume through its vertical suspension technique. It suits patients who desire notable rejuvenation with reduced scarring.

The Mini-Facelift (Soft Lift / S-Lift): The Lighter Option

The mini-facelift represents the lightest option among surgical facelifts. It is designed for patients with early sagging who wish to act before changes become more pronounced.

  • Indication: early to moderate sagging, loss of jawline definition
  • Typical age: 40 to 55
  • Duration: 1.5 to 2.5 hours, possible under sedation
  • Recovery: 8 to 12 days
  • Result: subtle, natural rejuvenation

The mini-facelift is ideal for patients who want to "stay ahead" of ageing without undergoing a more extensive procedure. However, it is not sufficient for correcting advanced sagging.

The Temporal Lift: Targeting the Eye Area

The temporal lift focuses specifically on the temple and brow region. It corrects temporal descent and brow ptosis, which give the eyes a tired or stern appearance.

  • Indication: brow ptosis, tired-looking eyes, temporal descent
  • Typical age: variable, from 40 onwards depending on facial structure
  • Duration: 1 to 2 hours
  • Recovery: 10 to 15 days
  • Result: open, refreshed eyes

The temporal lift is often combined with a cervico-facial facelift for comprehensive, harmonious facial rejuvenation. It also serves as a modern alternative to the forehead lift, which has now been largely replaced by Botox for the treatment of forehead lines.

How to Choose? The Bespoke Approach

The choice of facelift technique should never be made by the patient alone, based on an article or a video. It is the surgeon's role to analyse the anatomical characteristics of the face, assess the degree and location of sagging, and recommend the most appropriate technique.

Dr Weiss applies a systematic bespoke approach. During the consultation, he carefully examines the skin quality, the extent of fat loss, the condition of the SMAS and the facial proportions. He also listens to the patient's expectations, because the ideal result is one that matches the wishes expressed while remaining realistic and natural.

This philosophy is summed up in one phrase: "a little less is far better than overdoing it". A skilled surgeon will sometimes recommend a mini-facelift where the patient had envisioned a cervico-facial facelift, or conversely suggest a more comprehensive technique when a lighter procedure would not deliver satisfactory results.

Complementary Procedures

Whatever facelift technique is chosen, complementary procedures can optimise the result as part of a Complete Facial Rejuvenation:

  • Fat transfer (lipofilling) restores lost volume in the cheekbones, temples or hollow under-eye area
  • Blepharoplasty rejuvenates the eyes by treating excess eyelid skin or under-eye bags
  • Platysmaplasty corrects visible platysmal bands in the neck

These procedures, performed during the same session as the facelift, deliver a harmonious and cohesive result across the entire face.
