Double Chin: Effective Surgical Solutions for a Defined Jawline

A double chin is one of the most common aesthetic concerns raised during consultations. For many patients, it blurs the profile, obscures the cervico-mental angle and lends the face a heavy appearance that often has nothing to do with actual body weight. The good news is that today's surgical techniques can address this problem effectively and permanently, with natural results and a reasonable recovery. Dr Weiss welcomes international patients at his Paris practice for tailored assessment and treatment.

Why Does a Double Chin Develop?

A double chin results from several factors, often in combination. Genetics play a major role: some people have a familial tendency to accumulate submental fat, regardless of their weight. Ageing is also a factor: over the years, the neck skin loses its elasticity and the platysma muscle relaxes, creating the characteristic sagging appearance. Finally, bone structure can make things worse: a receding chin or a weak jaw projection reduces the angle between the face and the neck, making a double chin more prominent.

Understanding the precise cause of the double chin is essential in selecting the most suitable treatment. This is why Dr Weiss always performs a thorough analysis during the consultation, evaluating the thickness of the fatty tissue, skin quality, platysma muscle tone and chin projection. Each factor points towards a different solution.

Chin Liposuction: When Fat Is the Problem

When a double chin is primarily caused by excess submental fat, chin liposuction is the most direct and least invasive solution. This technique is ideally suited to younger patients whose skin retains good elasticity and will naturally retract after the fat is removed.

The procedure is performed through a tiny incision beneath the chin, just a few millimetres long, through which a fine cannula aspirates the excess fat. It takes approximately 30 to 45 minutes and can be carried out under local anaesthesia. Recovery is swift: wearing a compression bandage for a few days is sufficient, and normal activities can be resumed within a week. The final results become apparent after two to three months, once any residual swelling has subsided.

Platysmaplasty: Treating Muscular Laxity

When a double chin is accompanied by visible platysmal bands — those two vertical cords that appear down the centre of the neck — liposuction alone is not enough. The platysma muscle itself must be addressed. Platysmaplasty involves bringing the inner edges of the platysma muscle together and suturing them along the midline of the neck, recreating a firm muscular sling that redefines the cervico-mental angle.

This procedure combines naturally with liposuction for an optimal outcome: the fat is removed, the muscle is tightened, and the neck regains a clean, elegant contour. Platysmaplasty integrates seamlessly into a cervico-facial facelift when the sagging also affects the rest of the face.

Cervical Lift: When the Skin Has Lost Its Elasticity

In older patients or when skin laxity is significant, neither liposuction nor platysmaplasty alone can restore a youthful, taut neck. The skin, having lost its elasticity, will not retract sufficiently after fat removal. In such cases, a cervical lift is necessary to remove the excess skin and redrape the tissues.

A cervical lift is often part of a full cervico-facial facelift, because neck ageing rarely occurs in isolation — it is usually accompanied by loss of jawline definition and the development of jowls. Treating everything in a single operation produces a coherent, harmonious result, in keeping with the "Complete Facial Rejuvenation" approach favoured by Dr Weiss.

What If the Chin Bone Is Part of the Problem?

An often-overlooked factor in a double chin is insufficient chin projection. When the chin recedes, the distance between its tip and the neck is reduced, creating the illusion of excess tissue even when there is little surplus fat. In such cases, a genioplasty — a procedure to advance or reshape the chin bone — can be considered to restore proper profile proportions.

Dr Weiss systematically evaluates chin projection during every consultation. If a receding chin is contributing to the problem, he will explain this and may suggest an appropriate correction that can truly transform the profile.

Which Solution Is Right for You?

The choice of technique depends on your anatomy, age and the precise nature of the problem. Dr Weiss takes a fully bespoke approach: there is no one-size-fits-all solution. A 35-year-old patient with localised fat excess will not be treated the same way as a 60-year-old patient with generalised skin and muscle laxity. "Less is always better than too much" — this philosophy guides every decision, for a result that looks naturally yours.
