Your eyes are the first thing people notice about you. They are also, unfortunately, one of the first areas of the face to betray the passage of time. Heavy eyelids, under-eye bags and a permanently tired expression can project an image that bears no relation to how you actually feel. Blepharoplasty is the procedure that corrects these signs, restoring a fresh, open and rested gaze. The skin around the eyelids is the thinnest on the entire body, measuring barely half a millimetre in thickness compared to an average of two millimetres elsewhere on the face. This makes it particularly vulnerable to the effects of ageing: loss of elasticity, sagging and the appearance of fine lines. Add the constant mechanical stress of approximately 15,000 blinks per day, along with environmental factors such as sun exposure, smoking and lack of sleep, and it becomes clear why the eye area is often the first to show signs of age.
Over the years, the skin of the upper eyelids droops, forming excess folds that can eventually hang over the lashes and reduce the field of vision. The lower eyelids develop fatty pouches and a laxity that deepens the groove between the eyelid and the cheek, making dark circles more prominent.
Upper blepharoplasty is one of the most sought-after procedures in facial cosmetic surgery, and with good reason: it delivers a remarkable improvement with a relatively quick recovery. The procedure involves removing the excess skin that weighs down the upper eyelid, along with any small internal fat pockets if necessary.
The incision is placed within the natural crease of the eyelid. Once healed, it becomes virtually invisible, hidden in the fold that everyone has. The procedure is most commonly performed under local anaesthesia and takes approximately one hour. The result is a more open, brighter gaze with no artificial appearance whatsoever. Patients typically look as they did ten or fifteen years earlier.
Lower blepharoplasty addresses a different but equally ageing concern: under-eye bags and hollow dark circles. These bags are formed by orbital fat that gradually migrates forward through a weakened septum over time. They give the face a permanently tired appearance that neither rest nor cosmetics can correct.
Dr Weiss favours a conservative approach: rather than simply removing the fat, he repositions it to fill the hollow of the tear trough. This redistribution of volume creates a smooth transition between the eyelid and the cheek, avoiding the hollow-eyed look sometimes associated with older techniques. Depending on the individual case, the incision is made externally, just below the lash line, or via a transconjunctival approach (inside the eyelid), leaving no visible scar.
Blepharoplasty fits naturally into the philosophy of "Complete Facial Rejuvenation" championed by Dr Weiss. The eyes do not age in isolation: when the eyelids sag, the forehead, brows and jawline are typically showing signs of laxity as well. Rejuvenating only the eyes while the rest of the face remains marked would create an unnatural mismatch.
This is why blepharoplasty is often combined with a cervico-facial facelift or a temporal lift to reposition drooping brows. Fat transfer may also complement the procedure by restoring lost volume around the temples or cheekbones. The goal is always the same: a harmonious, natural result across the entire face.
Blepharoplasty offers one of the best result-to-recovery ratios in all of facial cosmetic surgery. Bruising and swelling are moderate and typically resolve within 7 to 10 days. Most patients resume their social activities within a week to ten days. Sutures are removed between the fifth and seventh day.
The final result can be fully appreciated after two to three months, once the tissues have settled and the scars have faded completely. The benefits are long-lasting: blepharoplasty results generally hold for ten to fifteen years.
As with every procedure, Dr Weiss places great importance on the pre-operative consultation. A thorough examination of the eye area assesses precisely what needs to be corrected: excess skin, fat pockets, brow position and skin quality. Every patient has a unique anatomy, and the technique must be adapted accordingly. "Less is always better than too much" — this philosophy applies especially to the eye area, where even the slightest over-correction would be immediately visible.
