Prominent ears are among the physical features that cause the most distress, particularly during childhood and adolescence. A source of teasing and self-consciousness, they can have a lasting impact on confidence. Otoplasty is the surgical procedure that reshapes the ear cartilage to achieve a more harmonious position and form. It is one of the few cosmetic procedures that can be performed from childhood, and the results are permanent.
Otoplasty encompasses all surgical techniques designed to correct abnormalities in the shape and position of the ear. In the vast majority of cases, it addresses protruding ears, but it can also correct ears that are too large (macrotia), asymmetrical, or lacking the natural fold of the antihelix.
Excessively protruding ears are caused by a cartilage folding defect. Either the antihelix is insufficiently defined, the concha (the central part of the ear) is overdeveloped, or both anomalies coexist. Otoplasty reshapes these cartilaginous structures to achieve natural-looking ears that sit closer to the head without appearing pinned.
Otoplasty is one of the rare cosmetic procedures that can be carried out from childhood. The ear cartilage reaches its near-final size around the age of seven. From this age, the operation is possible provided the child is the one requesting it — a crucial point, as the motivation must come from the child, not solely from the parents.
In adults, otoplasty can be performed at any age. Many patients consult in their twenties, thirties or even fifties, having spent their entire lives wearing hairstyles designed to hide their ears. It is never too late to correct this concern and finally feel free to wear short or tied-back hair.

Otoplasty is performed as a day-case procedure, usually under local anaesthesia in adults or light general anaesthesia in children. The incision is placed in the retroauricular sulcus — the natural crease behind the ear where it meets the skull. This placement makes the scar virtually invisible.

The procedure takes approximately one hour for both ears. Dr Weiss's philosophy applies fully here: a little less is far better than overdoing it. The goal is not to pin the ears flat against the head, but to give them a natural position in harmony with the facial proportions.
Recovery from otoplasty is generally simple and relatively pain-free. A compression bandage is placed around the head at the end of surgery and worn for the first few days. It is then replaced by a soft headband, worn at night for approximately one month to protect the ears during sleep.
Swelling and bruising are moderate and resolve within about ten days. Sutures are removed between the seventh and tenth day. Return to school or work is usually possible after about a week. Sport can be gradually resumed after one month.
The result of otoplasty is visible immediately, although the final shape is best appreciated after two to three months, once swelling has fully resolved and the cartilage has stabilised in its new position. The benefit is permanent — the ears do not re-protrude over time.
For patients, the change goes far beyond physical appearance. Many describe genuine psychological relief, the freedom to style their hair as they wish, and renewed self-confidence. This is especially true for children, who regain peace of mind at school and in their social relationships.
The ears contribute to the aesthetic balance of the face, even if we do not always think of them. Overly prominent ears can draw attention away from other facial features. Conversely, a successful otoplasty refocuses the gaze on the face itself — the eyes, the nose, the smile — rather than the ears.
