Volumetric Ageing: A Long-Overlooked Phenomenon

For decades, facial cosmetic surgery focused primarily on skin laxity. Facelifts aimed chiefly at tightening sagging skin and tissues. But our understanding of facial ageing has evolved profoundly: we now know that volume loss plays a role every bit as significant as sagging.

With age, the fatty compartments of the face atrophy unevenly. The temples hollow, the cheekbones lose their projection, the nasolabial folds deepen, and the lip contour thins. This fat depletion, combined with resorption of the underlying facial skeleton, gradually transforms a full, oval face into one that appears angular and drawn. Tightening the skin without restoring these volumes can only produce an incomplete result.

The Principle of Fat Grafting: Reinject Your Own Fat

Fat grafting — also known as lipostructure or lipofilling — involves harvesting fat from the patient (abdomen, thighs, hips), purifying it through centrifugation, and then reinjecting it in micro-droplets into the areas of the face that have lost their volume. The fundamental advantage of this technique is the perfect biocompatibility of the injected material: it is your own tissue, with no risk of rejection or allergic reaction.

Dr Weiss performs fat grafting using the Coleman technique, which ensures the best survival rate for the transplanted fat cells. The fat is harvested gently through low-pressure aspiration, then centrifuged to separate viable cells from oil and fluid. Only the purified fat cells are reinjected, in small quantities across multiple tissue planes, to maximise their take and integration into the surrounding tissues.

Areas Treated by Facial Fat Grafting

Facial fat grafting can restore volume in all the areas that hollow with time:

  • Under-eye area: to soften the hollow between the lower eyelid and cheek, often responsible for a tired look.

  • Lips: to restore natural volume without the overfilled look that some fillers can produce.
  • Chin and jawline: to refine the profile and strengthen the structure of the lower third of the face.

Fat Grafting vs Hyaluronic Acid: What Is the Difference?

Hyaluronic acid is the other major option for restoring facial volume. It is a resorbable dermal filler, injected in the consultation room without general anaesthesia, with immediate results. Its strengths lie in simplicity and reversibility. However, it has one significant drawback: it is absorbed within 12 to 18 months, requiring regular top-up treatments.

Fat grafting, by contrast, delivers a permanent result. Once the fat cells have taken — typically 60 to 80 per cent of the injected volume — they remain permanently integrated into the tissues. Fat grafting is therefore particularly appealing when the volumes to be restored are substantial or when the patient wants a lasting outcome without recurring injections. For lighter corrections or occasional maintenance, hyaluronic acid remains an excellent option.

Fat Grafting and Facelift: The Ideal Combination

Fat grafting truly comes into its own when combined with a cervico-facial facelift or a MACS lift. The facelift corrects sagging by repositioning drooping tissues, while fat grafting restores the lost volume.
